Life gets complicated. Whether you’re managing a behavioral health condition, navigating substance use recovery, or simply trying to hold things together while accessing the right services — you don’t have to figure it out alone.

Community Psychiatric Supportive Treatment (CPST), also known as Case Management, connects you with a dedicated support specialist who works alongside you. Not above you, not in front of you — alongside you. Together, you’ll identify what’s needed, build a realistic plan, and take the steps to get there.

This service is for both adults and youth, and it meets you in the environments where you actually live your life.

What your case manager does for you

Your case manager is your navigator, advocate, and coordinator — a consistent presence who knows your history, understands your goals, and actively works to remove barriers in your path. Case management is not passive check-ins. It’s hands-on, goal-oriented work that adapts as your needs change.

Substance use support (SUD)

For individuals navigating substance use recovery, a case manager provides focused, sustained support that goes beyond the walls of treatment — helping you reconnect with your community and rebuild your life from the ground up.

Recovery is not a single event. It’s a process, and that process requires ongoing support in the real world. Our case managers understand this and show up accordingly.

  • Guidance and accompaniment to treatment facilities

  • Active coordination with treatment providers throughout your recovery

  • Help re-establishing connections to community services post-treatment

  • Ongoing advocacy as you move through different stages of recovery

  • Support navigating legal, housing, or employment challenges related to recovery

Who this service is for

CPST is designed for individuals who are dealing with more than a weekly appointment can address. If you or your loved one is navigating a behavioral health condition, substance use recovery, or the complexity of multiple systems at once — case management provides the connective tissue that holds everything together.

  • Adults and youth, ages 5–64

  • Individuals with a behavioral health diagnosis requiring coordinated care

  • People in or seeking substance use treatment and recovery support

  • Those facing barriers to housing, employment, education, or transportation

  • Individuals transitioning from inpatient, residential, or higher levels of care

  • Youth and families involved with school systems, courts, or child protective services

What makes this different

Case management at Cornerstone is not a binder full of referrals. It’s an active, working relationship between you and a specialist who knows your name, knows your goals, and stays in your corner through the hard parts.

Services are delivered where you are

Most case management happens in your natural environment — your home, your neighborhood, your community. This is intentional. The barriers people face are rarely solved in an office.

The plan belongs to you

Your case manager doesn’t arrive with a predetermined plan. Goals are identified by you, and the work is structured around your definition of progress and self-sufficiency.

Coordination is built in

Your case manager actively communicates with your broader care team — physicians, therapists, social workers, schools, and more — so nothing falls through the cracks and everyone is working from the same page.

At a minimum, once per week for at least 90 minutes. Many clients work with their case manager for extended periods as needs evolve. Scheduling is determined collaboratively between you and your case manager.

Where do sessions take place?2026-05-22T17:33:28-04:00

Primarily in the community — wherever is most practical and meaningful for you. Sessions can also occur in the home or at our offices based on your preference and needs.

Service providers, parents and guardians, adult clients, school administrators and counselors, courts, child protective services, and care management entities (CMEs) can all submit referrals.

Is this covered by Medicaid?2026-05-22T17:34:19-04:00

Yes. We accept Ohio Medicaid and are contracted with CareSource, Anthem, Humana, Molina, United Health, Amerihealth Caritas, Buckeye, OhioRise, Aetna, and Paramount. Self-pay options are also available and discussed before intake.

All services are provided by trained, supervised direct staff who meet the full requirements of Ohio Mental Health and Addiction Services, including background checks and ongoing supervision.
